Following a review of the information contained within this document please indicate

which level survey you require (Level 1, Level 2 or Level 3) and sign and date the below to confirm you understand the fee structure and the information which will be included within your chosen survey.

Survey reports will be issued within 10 working days on completion of the on-site survey (unless otherwise agreed) following full payment of our invoice. Cancellations must be made 48 hours prior to the scheduled inspection to avoid charges. Please refer to Appendix A, Section 20.
By signing this document, you agree that you have read, understood and accepted the Terms of Engagement, the Terms and Conditions (Appendix A), the variances in the level of service included within this document, and the below statements,

• I authorise The Welsh Surveying Co Ltd to immediately commence works on the report and I accept that once the report has been provided to me, I will lose my right to cancel during the 14-day ‘cooling-off’ period (as provided by The Consumer Contracts (Information, Cancellation and Additional Charges) Regulations).

• I accept that The Welsh Surveying Co Ltd may in some instances pay a referral fee to a third party in connection with this transaction.

• I authorise The Welsh Surveying Co Ltd in accordance with these terms and its privacy policy.
